Paravertebral spine blockade

Procedure goal

Paravertebral blockade (cervical, thoracic, lumbar spine) is a therapeutic manipulation, the essence of which is to inject a medicine into the zone of greatest pain and neurodynamic disorders in order to restore local homeostasis and reduce neurodystrophic changes in muscles, ligaments, joints.

Areas of administration are trigger points for overloaded joints and strained back muscles, as well as anatomical areas of the passage of nerves and location of their plexuses. As a rule, the pain syndrome is accompanied by neurotization of the patient, depressive reactions, asthenohypochondriac and conversion disorders; it also disrupts the quality of life.

Paravertebral blockade is used to eliminate pain near the spine, preventing the disease from becoming chronic: its pathological cycle is interrupted and recovery processes are started. A medicine is injected into the pathological focus, and its depot is created. The injection is done near the spine where the pain is felt the most. That's where the nerve endings pass. After the blockade, the patient's edema disappears and the trophism of the nerves improves.

Paravertebral blockade is also practiced to prevent muscle spasms, relieve their pathological tension.

Indications

Paravertebral blockade is recommended for pathologies of the spine and the musculoskeletal system, when other methods of pain relief have failed.

Reflex pain and radicular syndromes;
intercostal neuralgia;
muscular-dystonic syndrome;
cervicalgia, cervicocranialgia;
pinched nerve;
arthrosis;
spinal hernia;
radiculitis;
chronic lumbar pain;
osteochondrosis;
muscle inflammation;
chronic back pain;
Chronic neck pain.

Contraindications

Paravertebral blockade is contraindicated in case of excessive sensitivity of the patient to the components of the used medicine and analgesics. The injection is not done in case of damage to the skin and infectious processes. Other contraindications:

pregnancy;
reduced blood pressure;
severe pathology of the cardiovascular system;
lactation period;
renal failure;
liver failure;
high body temperature;
suppuration in the pleural cavity;
neoplasms in the paravertebral space;
epilepsy;
blood clotting problems.

This method of therapy is not indicated for patients undergoing heparin therapy. With caution, the blockade is done with curvature of the spine, deformities of the chest and gross violations of posture. If there is scar tissue or inflammation in the area where the injection is planned, it is recommended to choose another method of therapy.

Paravertebral blockade is also not done in osteoporosis, since the patient's condition in this case may worsen. In patients with osteomyelitis, there is a risk of getting the medicines used into the nerve tissue, which may result in an abscess.

Paravertebral block is contraindicated in patients in a state of shock.
